Making the decision about what type of care you need can be overwhelming as there are many options to choose from. At Bromley Healthcare Care at Home, we do our best to simplify the process, keep you as informed as possible and help you to make the right decision. Our team work with you and your family to create a tailor-made care plan to best suit your needs.
Assessing your care needs
One of the first steps to consider is completing a needs assessment through your local authority. This is where a trained professional will complete an assessment to determine which of your needs the council can support with. This takes into consideration factors such as:
- Your needs and how they impact on your wellbeing, for example, do you need help getting dressed or support to get to work.
- The outcomes that matter to you, for example, whether you are lonely and want to make new friends.
- Your individual circumstances, for example, whether you live alone or whether someone supports you.
The benefits of care at home
Being able to stay in your own home whilst receiving care is a great option and one that is best for many people. Introducing extra support at home is a good way to ease yourself into the process of care whilst still being in control of your life. Home care is also a great value for money as it’s a fraction of the cost of most residential care homes.
Regulated care for your peace of mind
At Bromley Healthcare Care at Home, we pride ourselves on giving you the best service possible and are externally regulated through the Care Quality Commission (CQC). The CQC make sure health and social care services provide people with safe, effective, compassionate, high-quality care and they provide us with feedback and improvements where necessary. At the last CQC inspection, we were awarded a ‘Good’ rating.
*Please note we were previously known as Link Care Nursing Agency until being acquired by Bromley Healthcare in August 2021, until our new report is published our rating is still listed under Link Care.
Agreeing a Care Plan
During the initial assessment, our supervisor will agree a ‘Care Plan’ with you which will detail the tasks you would like us to do on a regular basis. However, if your care needs change over a period of time, we will be happy to revisit the ‘Care Plan’ and make the relevant changes as requested.
